Registration Information

Roman Catholic Schools in the Archdiocese of New York base their educational purpose and all their activities on the Christian teaching of the essential equality of all persons as rooted in God’s love.

Thus, with discrimination so repugnant to their nature and mission, Catholic Schools in this Archdiocese do not discriminate on the basis of race, color, national and ethnic origin, and gender in administration of educational policies, admission policies, scholarship and loan programs, and athletic and other school administered programs.

This policy is subscribed to by all Catholic elementary and secondary schools in the Archdiocese of New York whether owned or operated by the Archdiocese, the parishes of the Archdiocese, Regional Boards, or religious communities with the Archdiocese.

The process for admission to Our Lady of Perpetual Help School is as follows: parents must complete the school application form and provide all required documents. Following an interview, an evaluation of the materials, and a two day student visit, the parent will be notified in writing about the status of the child.
Our Lady of Perpetual Help School gives preference in admission first, to siblings of currently enrolled students; second, to Catholic students whose parents are active members of the parish; third, to Catholic students whose parents are active in another Catholic parish; fourth, to non-Catholic students.

Admissions Application

Application for the 2018 - 2019 school year at Our Lady of Perpetual Help School has begun.  You may pick up the Application Packet at the School Office.   This packet, along with all required documents, is to be returned to the Main Office when you apply.  Registration will take place only after all paperwork is complete, a student visit (see below) has taken place, an interview and testing have occurred, and your child has been granted admission.

Application Check List

Requirements for application are:                                                              

•    $25.00 application fee (Non-refundable)   

•    Birth Certificate

•    Baptismal Certificate (if Baptized in the Catholic Church)

•    Social Security Number

•    Grades One to Eight - Report Cards from current year and last year’s as well as Standardized Reading and Math Scores

•    Kindergarten - Latest Report Card

•    Copy of any testing or evaluations conducted

•    Principal’s name, Teacher’s name, and Guidance Counselor, if applicable, with phone numbers and extensions

•    Letter of reference from current teacher or principal - when requested

•    A 2-day visit (consecutive days) for all children entering 2nd  through 8th Grade

•    A 1-day visit for all children entering 1st Grade

•    A ½ day visit (AM) for all children entering Kindergarten         ½ day visit (PM) for all children entering Pre-K

 

Requirements for registration are:

•    Immunization Health Card

•    $100.00   Registration Fee (Non-refundable)   

 

The minimum age requirements for registration are:

•    Pre-Kindergarten - four years old by December 31, 2018

•    Kindergarten - five years old by December 31, 2018

•    First Grade - six years old by December 31, 2018

 

The hours of the school day for all children in Kindergarten - Grade 8 are from 8:15 AM to 3:00 PM

 

The hours of the school day for all children in Pre-K are - AM session 8:15 – 11:45           Full Day 8:15AM-3:00PM

 

Early Morning Drop Off (at 7:30 AM) and After-School care (from 3:00 -6:00 PM) are available for additional fees.
